OSCA Community Builder Award Nominations Form

This is a two-page nomination form. Reasons why the Nominee should be considered for an OSCA Community Builder Award should be included on page 2.
Note that the Nominator and Seconder must be residents of OOS, but the Nominee need not be.
Current members of the OSCA Board and Committees, as well as OSCA Staff, are not eligible for this award while in office.
